Step 1 – Get Your MyChart Activation Code

You usually have 3 ways to get the activation code:

  1. From your doctor’s office – ask them to print or email your MyChart activation code during your visit.
  2. From a MyChart sign-up email or text – some clinics send this automatically after your appointment.
  3. Request one online – go to the MyChart login page for your hospital/clinic and click “Sign Up Now.”

Step 2 – Go to the MyChart Sign-Up Page

  • Open the MyChart website or download the MyChart app on your phone.
  • Click Sign Up Now.

Step 3 – Enter Your Information

  • Activation Code – type in the code from your doctor’s office or email.
  • Date of Birth – your date of birth if it’s for your own account; your child’s if you’re setting up for them.
  • Other info – some systems ask for your ZIP code or medical record number.

Step 4 – Create Your Username and Password

  • Pick something easy for you to remember but secure.
  • You may be asked to set up security questions for password recovery.

Step 5 – Set Up Proxy Access (for Your Child)

  • Once your account is active, log in and look for “Request Access to a Family Member’s Record” or “Proxy Access”.
  • Fill in your child’s information.
  • The clinic may approve instantly or may take 1–2 days for staff to confirm.

Step 6 – Log In and Explore

  • You can now view visits, message your child’s care team, check immunizations, request appointments, and more.
  • If you download the MyChart app, you can log in anytime from your phone.
